Description of Position

The Practical Nurse is a member of the clinical staff who provides medical expertise to the multidisciplinary teams responsible for the delivery of effective services to the individuals served to promote their health and wellness. The Practical Nurse is an effective member of the team with experience in providing a variety of health and wellness services to persons with behavioral health challenges, development and intellectual disabilities, including development of comprehensive assessments and treatment plans, delivery of a wide range of clinical and supportive services, and working collaboratively with the treatment team and other service providers, including DDDS and DSAMH. The Practical Nurse utilizes evidenced-based practices, including Motivational Enhancement Therapy, Trauma-Informed Care, and medication assisted treatment, to effectively intervene and support individuals served.

Education and Experience

  • Graduate of an accredited School of Nursing
  • Minimum one year of clinical experience (Preferred)

Professional Credentials/Certificates

  • Must have and maintain current licensure as a Licensed Practical Nurse in Delaware

Position Requirements

  • Must receive and maintain a satisfactory urine drug screen, abuse registries review, and criminal background investigation.
  • CPR Certification
  • Pre-employment Tuberculin Skin Test and CPR Certification

Responsibilities

  • Assists in addressing health care service needs of individuals served and ensures that these service needs are addressed in collaboration with the Nurse.
  • Completes all required medical assessments pursuant to best practices, contractual obligations, and licensure requirements.
  • Ensures that an individual’s health care services adhere to the best practice, licensure requirements, and are consistent with the policies, procedures, and protocols of contractors and payers of services.
  • Assists in the delivery of effective wellness and medical services in accordance with policies and procedures; state and federal regulations; and extant professional, ethical, and legal principles in consultation with the health and wellness leaders.
  • Maintains an effective therapeutic alliance with individuals served.
  • Participates in care coordination activities and meetings.
  • Completes all required documentation, audits, reviews, and reports in a timely manner relative current and ongoing health care service needs.
  • Communicates and coordinates with individuals receiving services, healthcare providers, family members or legal guardian regarding individuals’ health care needs.
  • Utilizes the required Electronic Health Record to record and document all health care encounters and interactions.
  • Abides by state and federal regulations, policies, and procedures related to confidentiality, patients’ rights (PM 46), and exposure and infection control.
  • Participates in performance improvement activities to evaluate and resolve problems related to risk management, utilization review, and quality assurance.
  • Provides after hours on-call support to program staff.
  • Performs other duties as requested or assigned, verbally or in writing.

  • Clinical Knowledge: Possesses nursing and medical assessment and intervention skills, specifically in utilizing evidence-based practices and implements these skills in a professional manner.
  • Problem Solving: Ability to tell when something is wrong or is likely to go wrong. It does not involve solving the problem, only recognizing there is a problem.
  • Inductive Reasoning: Keenly observes and gets to know clients, anticipates how client may react.
  • Written Communication: Content expert at documentation, notes are professional, descriptive, free of grammatical errors
  • Oral Communication: Ability to relate and convey concise directions to clients and all who are part of Connections team. Ability to advocate for client with professionalism and compassion when talking with external support members.
  • Quality Control: Demonstrate accuracy and thoroughness with understanding of clinical documentation and the requirements for state standards and expectations. As well as details related to appointments, deadlines, goals, and client needs
  • Adaptability: Adapt to changes in the work environment, manage competing demands and demonstrate ability to deal with frequent change, delays or unexpected events and ability to work under emergency condition.
  • Stress Tolerance: Recognize crisis situations and react to such in a calm, mature manner; accept criticism and deal calmly and effectively with high stress situations; and the ability to identify, react to, and report security concerns
