Description: Employee Work Profile and Job DescriptionPosition Title: Nurse Full Time: Yes Reports to: RN Supervisor Direct Reports: None Department/Program: Blackbird Landing Group Home Location: Townsend, DE Description of Position This is a professional nursing position which requires thorough knowledge of commonly used psychotropic and health maintenance medications, an understanding of the special medical needs of adults with severe and persistent mental illness, substance abuse conditions or cognitive disabilities who may have co-occurring substance use disorders, physical disabilities, debilitating medical conditions, developmental disabilities, or infectious diseases.

Must have the ability to function independently and as part of a team in carrying out the treatment plans and physicians’ orders in community settings.

Education and Experience * Bachelor’s Degree in Nursing or equivalent.

Professional Credentials/Certifications * Delaware Registered Nursing License in good standing or Compact License Position Requirements * Certification in First Aid and CPR* Mandt* Valid driver’s license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions.* Must be available to work the hours 8:00am to 4:00pm.

Responsibilities * Participates in treatment planning and case conferences with other staff of the community support program and with other agencies.* Completes all assigned tasks from the daily schedule which may include the following: 1.

maintaining a therapeutic alliance with clients; 2.

referring and linking clients to all needed services provided outside the agency and following up to see that said services are delivered; 3.

coordinating the provision of emergency services and hospital liaison services when a client is in crisis; 4.

coordinating overall independent living assistance services and working with community agencies to develop needed resources including housing, employment options, and income assistance.

5.

providing support and consultation with client’s family; 6.

providing in-home services including ADL training, medication training, social support, and home management assistance.

7.

providing supportive counseling to clients.

* Documents and keeps other staff, clinical coordinators, and Team Leader/Program Director informed of all changes in the consumer’s mental or social status.* Provides nursing care and gives guidance to consumers and staff in the management of consumers’ physical and mental health needs.* For a designated caseload of individuals, completes and records vital signs at least monthly, metabolic screening at least once every 90 days, injections and other treatment as ordered, and alerts the prescriber of any changes or issues.* Manages the storage and administration of medications and treatments, ensuring compliance with applicable licensing regulations and statutes, including the Delaware Nurse Practice Act and Delaware Board of Pharmacy Regulations pertaining to Pharmaceutical Service in Health Care Facilities.* Manages the implementation of the infectious disease control program, including prevention, screening and surveillance, treatment and hazardous waste management.* Maintains the primary relationship with the pharmaceutical contractor and assures compliance with all laws and regulations related to the storage, administration and disposal of medications.* Maintains the primary liaison with the consulting dietitian (if applicable) and ensures that therapeutic diet orders are carried out.* Maintains the primary liaison with the consulting providers of primary medical and ancillary services such as podiatrists, physical therapists and occupational therapists and ensures that orders are carried out.* Provides training and supervision to unlicensed personnel for the delivery of medical and personal care services to the residents in the areas of basic nutrition, observation and reporting of signs and symptoms of common diseases and conditions; assistance with hygiene and housekeeping tasks, vital signs measurement, catheter care, unsterile dressings and soaks, cold and heat applications, bowel and bladder training, incontinence care, use of assistive and prosthetic devices and care for dying patients.* Provides supervision for unlicensed personnel in assistance with self-medication.* Provides training and supervision of unlicensed personnel in Advanced Directives, CPR, First Aid and procedures to be followed in life threatening and non-life threatening medical emergencies.* Performs treatments and administers care in accordance with physician’s orders and protocols and/or as provided for by the standing orders or procedures of the facility.* Completes medical and physical assessments for all assigned clients at admission and at least annually thereafter.* Ensures that all consumers have annual histories and physicals, follow-up care as ordered and any necessary dental, ophthalmology and podiatry evaluations and follow-up care.* Ensures, through weekly chart and face-to-face evaluation of each consumer, that all medical orders are followed and that 100% of all laboratory and other diagnostic tests and workups are completed and documented.* Attends and participates in treatment team meetings, staff meetings, shift reports, training sessions and other meetings as designated by policy and/or as assigned.* Conducts symptom education and/or medication training, particularly as these relate to the communication, prevention, and treatment of infectious diseases.* Documents and keeps other staff members informed of all changes in consumer mental status and physical health.* Assists and provides input to all team members, staff and physicians.* Maintains inventory of pharmacy stock and individual medications, orders new medications, receives incoming medications, and ensures that medications are ordered, stored and administered in accordance with all applicable laws, regulations and policies.* Ensures that all routine and follow-up medical appointments are made, kept and documented.* Communicates to other staff about medication changes and changes in consumer condition which may impact on behavior, treatment and/or restrictions.* Carries primary or secondary beeper as assigned and responds to beeper calls in accordance with program directives.* Provides education and works as an advocate for the client in the community.* Performs other duties as requested or assigned, verbally or in writing.

Knowledge, Skills, and Abilities * Clinical Knowledge: Possesses understanding of assessment and intervention skills, specifically evidence-based practices.

Implementation of these skills is done in a professional manner.* Problem-Solving: The ability to identify a problem, or to identify situations that could lead to problems, in an effort to intervene quickly and effectively.

This may not involve resolving the issue alone, only recognizing the problem, and demonstrating sound judgment by gathering and analyzing information* Critical Reasoning: Excellent observation skills and the ability to combine pieces of information to form general rules or conclusions, as well as extracting themes or patterns from seemingly unrelated pieces of information* Communication: Ability to convey complicated clinical and professional information to various audiences, in both written and oral form.

Understanding the context in which information exists, and the ability to temper opinions diplomatically, all in an effort to model mature, positive, and appropriate behavior.

Written communication is edited for spelling and grammar, and presents numerical and scientific data appropriately in clear, concise, and accurate manner.* Adaptability: Quickly adapts to changes in the work environment, manages competing interests, and demonstrates the ability to deal with frequent changes, delays, or unexpected events* Stress Tolerance: Recognizes crisis situations and reacts to such in a calm, mature, professional manner.

Accepts criticism and feedback, and is able to deal calmly and effectively with high stress situations.
