Admission Coordinator – Medical Records


This is a Full-time position in Dover, DE posted November 22, 2021.

Position Summary : This position is responsible for the overall coordination of the referral and admissions process.

Interacts closely with facility representatives, discharge planners and other agencies to facilitate the initiation of care.

Essential Duties:
Demonstrates knowledge and skill of the agency complaint, abuse, neglect and exploitation process, state/federal guidelines and DOH Incident Reporting Policy
Knowledge in all Home Health processes, with basic knowledge of Skilled Care and Personal Care Service processes
Perform processing of physician orders, including initial certification signatures, recertification signatures, and other orders
Receive and process authorizations
Manage 485’s and ensures agency is compliant
Processes medical records from referral through discharge
Fax lab reports and other reports to physicians or other agencies involved in client’s care including resolving unsigned orders
Oversee scheduling for patient from admission to discharge
Resolve paper and software workflow from admission to discharge
Receive referrals by phone, fax, email, or in-person for designated department
Perform all medical record organization, including assembling new charts, performing initial audit, maintenance of chart organization and filing, and performing periodic audits for completeness of medical records
Assures all paperwork for department is properly filed and maintains par levels of forms
Maintains a high degree of confidentiality at all times due to access to sensitive information
Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
Performs other duties as assigned
Position Requirements & Competencies:
Be eighteen (18) years of age or older with high-school diploma
At least one (1) year of experience in homecare, hospice and/or healthcare related field.

Experience in home health strongly preferred
Ability to complete necessary documentation appropriately, legibly and timely
Ability to handle stressful and delicate situations
Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
Must have reliable transportation, valid driver license and state required insurance
Must undergo background check and pre-hire drug screening
